Required for all applicants.
Corporation / Partnership Information
If all information on this form is contained in another document, it may be substituted.
Individual / Sole Proprietor Application
If all information on this form is contained in another document, it may
be substituted.
Abbreviated Personal Financial Statement
Required for all sole-proprietors, general partners and guarantors. If all information on this form is contained in another document, it may be substituted.
Business Plan
We require a business plan. Since these take
many forms, we have included general guidelines in our application. If another document already exists, it may be substituted.
Supporting Documentation
Bank Statement
A copy of your most recent bank (or other financial institution) statement to verify the available funds listed on your financial statement.
Tax Returns
A copy of prior year’s tax return.
Additional Information
Any additional information you feel will be helpful for the landlord to analyze your application, such as photos of existing stores, business plan, etc.
Occupational License
Depending on services offered, applicants may be required to have a government issued license.
